Parents, Captain Shreve High School needs your help to make sure we are able to provide the right programs and resources for your child’s academic success. Completing this survey will confirm our school receives the full amount of federal and state funding and will make sure your child receives all the educational services and programs they are entitled to receive. Because we are a district that provides free breakfast and lunch to all students, we have not captured some critical information that is used by federal and state agencies to provide schools with funding based on family income. The information can also be used to determine if your child qualifies for ACT vouchers or other resources. All data collected is confidential. If your student or household is currently receiving SNAP, TANF, Medicaid, or Disaster SNAP, completion of this survey is not necessary. If you have any questions, please call the school for more information.
